I (name) apply for permission to issue a writ of possession to give possession to the (party) of the land described as (specify)
I produce herewith, in accordance with rule 83.13
(i) a copy of the notice issued by the Secretary of State for the Home Department dated (date);
(ii) a copy of the prescribed from of notice to quit (the eviction notice) dated (date) and served on the tenant (or in the case of joint tenants all tenants) on (date) (attached to this request. marked “A”;
(iii) proof of service of the notice of eviction and end of tenancy in the prescribed form, marked “B”.
I produce herewith, a schedule containing (deleted any paragraphs which do not apply)
(a) the name and address of the claimant and the address for service of documents if different;
(b) the full address of the property for which execution is sought (if the premises of which you seek possession are part of a building identify the part eg Flat 3, Rooms 6 and 7);
(c) the persons named in the notice issued by the Secretary of State;
(d) the names of the tenants of the property for which execution is sought;
(e) the names of any other persons in occupation of the property at the date of service of the notice to quit;
(f) the names of any other current occupiers of the property for which execution is sought and their status and the means by which they have received notice of the claimant’s intention to evict them;
(g) names and details of any persons in the property that are under the age of 18 and their relationship to the persons named in the notice issued by the Secretary of State or any other tenant;
(h) a copy of any written tenancy agreement in relation to the property for which eviction is sought;
(i) any other information which may be of relevance to the court.
I attach a writ in form No. 66.
