Abstract
Purpose This study aims to explore theexperiences of professional carers for people with intellectual
disabilitieswho were ‘‘at risk’’ of hospital admissionand to identify factors which were helpful or restrictive
in the supportreceived from health-care servicesduring this time.
Design/methodology/approach Semi-structured interviews were conducted with eight professional
carersfrom NHS trusts. Data was analysed usingthematic analysis.
Findings Three main themes and 11 subthemes were identified. The first theme discusses the
processes implemented to facilitate change while the person was at risk of hospital admission. The
second theme identifies social and environmental factors increasing risk of hospitalisation. The third
theme wasthe support received by staff.
Originality/value To the best of the authors’ knowledge,this study was the first to gather and analyse
data on carer’s experiencesin supporting an individual with intellectualdisability who is at risk of hospital
admissionwithin the locality of Greater Manchester.

Introduction
In 2011, a BBC documentary uncovered abuse towards individuals with intellectual
disability (ID) and autism, occurring at Winterbourne View private hospital (Plomin, 2013).
This fuelled investigations into the care of adults with ID, of whom estimates suggest
there are 1.5 million individuals living in the UK (Office for National Statistics, 2024). The
NHS initiated an urgent response to transform the quality of care of these individuals,
both within inpatient settings and in the community (Bubb, 2014). The Care and
Treatment Review: Policy and Guidance document (NHS England, 2015a,2015b,2015c;
NHS England, 2017) was developed as a part of an initiative towards the Transforming
Care Agenda, which states it aims to improve the care and quality of life for people with
an ID and/or autism, as well as enhancing community capacity and reducing
inappropriate admissions to hospitals. Within this initiative, the term “dynamic register” is
used to describe a compilation of records, held by Clinical Commissioning Groups (now
“Integrated Care Boards”) to identify individuals with an ID and/or autism, who have
mental health problems and/or challenging behaviour within their locality, according to
risk of admission to hospital settings.
