The forms prescribed by the Mental Health (Hospital, Guardianship, Community Treatment and Consent to Treatment) (Wales) Regulations 2008 (SI 2008/2439) (W212) and the Mental Health (Hospital, Guardianship, Community Treatment and Consent to Treatment) (Wales) (Amendment) Regulations 2012 (SI 2012/1265) (W158) for use in Wales by mental health professionals when performing functions under the MHA 1983 are as follows.

Form Purpose

HO1 Section 2 – application by nearest relative for admission for assessment
HO2 Section 2 – application by an approved mental health professional for admission for assessment
HO3 Section 2 – joint medical recommendation for admission for assessment
HO4 Section 2 – medical recommendation for admission for assessment
