post-COVID in 2024? A mixed-method
systematised review

Abstract
Purpose –Since COVID-19, many services have burgeoned within the UK, but what about sexual
minorities? Sincethe last review, there are appropriatetherapies, but there is often inadequateresearch.
The purpose of this mixed-methodreview synthesis looking into the efficacy of psychologicaltherapies
for sexualminorities. Seven studies were found in total.
Design/methodology/approach –A mixed-method review synthesis, three studies looking into the
efficacy of psychologicaltherapies for sexual minorities and four studiesaddressing the experiences of
sexualminorities partaking in psychologicaltherapies were identified.
Findings –These included three quantitative and four qualitative studies. The minority stress
hypothesis is used to formulate problems, but challenges remain to confidentiality and privacy in
this context. Therapists still operate within th e heteronormative framework, discounting
intersectionality in therapy conversations.
Research limitations/implications –Most studies have had low retention rates since 2021. It shows
that minoritystress needs to be accounted for at the ethics committeeand research delivery levels.
Practical implications –Applying a heteronormativeframework to sexual minorities is not working. An
alternativeprogress world view is needed.
Social implications –Health-care cliniciansstrive for equitable care. Unfortunately, using an equitable
health service scaleadapted from Levesque et al. (2013), the rating is 3 out of 6. More work is needed to
improveservices.
Originality/value –Some services are reporting much improvementpost-pandemic. Sadly, this is not
the casefor sexual minorities. Individualand systemic barriers remain.

Background
It is well established in the literature that the intersections of mental health and sexual
minorities overlap with other intersects of social class, ethnicity and religion, among others, all
operating within the personal, cultural and social domain. It is evident in sexual minor ity
outcome research that lesbian women and bisexual men have lower recov ery rates and higher
levels of dissatisfaction compared to other sexual minorities and straigh t populations
(Blosnich, 2017). In the end-session data collection for monitoring depression and anxiety,
they score at the severity end of the scale and for functioning levels in th e national health
service (NHS) service Increased Access to Psychological Therapies (IAP T) (Rimes et al.,
2018).
