• If you wish to act as a litigation friend for a party
to the proceedings then you must complete this
certif‌icate and f‌ile it with the court.
• This does not apply if you wish to act as a
litigation friend for the person to whom the
application relates. You can apply for an order
to be appointed as the litigation friend for the
person to whom the application relates (where
they are party to the proceedings) by f‌iling a
COP9 application notice.
• Please continue on a separate sheet of paper
if you need more space to answer a question.
Write the case number, your name, the name of
the person to whom the application relates, and
the number of the question you are answering on
each separate sheet.
