- By a judgment of (name of court of origin and other part of the United Kingdom) dated (date) it was ordered that (summarise terms of Judgment and particularly non-money terms).
- [I refer to] [there is shown to me] a bundle marked [“A”] containing
(a) a certified copy of the said judgment issued by the said court under Schedule 7 to the Civil Jurisdiction and Judgments Act 1982, and
(b) a certificate under paragraph 4(1)(b) of the said Schedule not more than six months before the date of this application stating that the conditions specified in paragraph 3 of the said Schedule are satisfied in relation to the judgment.
- To the best of my information or belief the usual or last known address;
a) of the claimant is (address), and
b) of the defendant is (address).
- The claimant accordingly applies for registration of the judgment in this Honourable Court under section 18 of and schedule 7 to the Civil Jurisdiction and Judgments Act 1982.
