  1. By a judgment of (name of court of origin and other part of the United Kingdom) dated (date) it was ordered that (summarise terms of Judgment and particularly non-money terms).

  1. [I refer to] [there is shown to me] a bundle marked [“A”] containing

(a) a certified copy of the said judgment issued by the said court under Schedule 7 to the Civil Jurisdiction and Judgments Act 1982, and

(b) a certificate under paragraph 4(1)(b) of the said Schedule not more than six months before the date of this application stating that the conditions specified in paragraph 3 of the said Schedule are satisfied in relation to the judgment.

  1. To the best of my information or belief the usual or last known address;

a) of the claimant is (address), and

b) of the defendant is (address).

  1. The claimant accordingly applies for registration of the judgment in this Honourable Court under section 18 of and schedule 7 to the Civil Jurisdiction and Judgments Act 1982.
