1. 3. I was present and saw (name of grantor) duly execute the (document) at the said time on the said date and I duly attested its execution.

4. (Name of grantor) resides at (state address at time when affidavit is sworn) and his occupation is (describe).

5. The name (name of deponent) signed on the (document) as being the name of the witness attesting its due execution is in my handwriting.

6. Before the execution of the (document) by (name of grantor) I fully explained to him its nature and effect.
