1. The document[s] exhibited hereto and marked “A” [is] [are] [a true copy] [true copies] of a bill of sale and of every schedule or inventory annexed thereto or referred to therein and of every attestation of its execution as made and given by (name of grantor or assignor) on (date of execution).
2. The bill of sale was made and given by (name of grantor or assignor) at (time of day) on (date).
3. I was present and saw (name of grantor or assignor) duly execute the bill of sale at the said time and on the said date and I duly attested its execution.
4. (Name of grantor or assignor) resides at (state address at time when affidavit is sworn) and his occupation is (describe).
5. The name (name of deponent) signed on the bill of sale as being the name of the witness attesting its due execution is in my handwriting. (Where there is more than one witness, adapt paragraph 5 accordingly, including the name, address and description of the other witness.)
