IT IS ORDERED THAT:-

1. This Order is made under rule 19.11 and applies to claims which give rise to the following common or related issues of fact or law (hereinafter called “the GLO issues”)

(specify here the issues which will identify the claims to be managed as a group under this Order)

2. Such claims are to constitute and be known as “the. . . . . . . . . . . . . . . . . . Group Litigation” and all documents or bundles of documents filed or served in respect of claims to which this Order applies are to be endorsed in the top left-hand corner “. . . . . . . . . . . . . . . . . . . . .Group Litigation Order”.

3. The initial claimants whose claims are the subject of this Order are those set forth in the title to this Order and in Schedule 1 thereto. The claim numbers of the claims issued by such claimants and the names of the solicitors representing such claimants are as set forth in the said Schedule opposite the name of each individual claimant.

4. There be established a group register on which the claims managed under this Order will be entered.

5. Such register shall, subject to further directions by the management court referred to below, contain details of the claims which are the subject of this order and shall include:-

(a) the full name, address and postcode of each claimant and of any litigation friend of such claimant

(b) the date of birth of the claimant

(c) the claim number of the claimant’s case and the date of commencement of the claim

(d) the name and address, reference, DX, fax and telephone number of the solicitors on record for the claimant

(e) the number and date of any notice of funding for the claimant

(f) the national insurance number of the claimant

(g) the date of entry of the claimant on the register

(h) the date on which any claimant ceases to be on the register

6. Such register shall, subject to further directions by the management court, be maintained by the Lead Solicitors referred to in paragraph 9 below.
