AND UPON READING Probate of the Will and Codicil / Letters of Administration of the above deceased granted on (date) to (name)

IT IS ORDERED that:

(1) under section (1)(a) of the Inheritance (Provision for Family and Dependants) Act 1975 there be made to the Claimant out of the net estate of the deceased monthly payments of £ each as from the date of the death of the deceased until the Claimant re-marries the arrears of such payments to be made on or before (date)

(2) under section 2(3) of the Inheritance (Provision for Family and Dependants) Act 1975 the property specified in the attached Schedule (being part of the net estate of the deceased) be set aside for the above monthly payments to be made out of the income from it of (name)

() under section 2(1)(a) of the Inheritance (Provision for Family and Dependants) Act 1975 there be made to the Claimant during her widowhood out of the net estate of the deceased quarterly payments equal to one half of the income of the net estate such quarterly payments to be made on 1st January 1st April 1st July and 1st October in every year the first payment to be made

on (date) and the last payment to be made on (date)

() under section 2(1)(a) of the Inheritance (Provision for Family and Dependants) Act 1975 there be made to the Claimant monthly payments equal to the whole of the income of that part of the net estate of the deceased which is specified in the attached Schedule

() under section 2(1)(b) of the Inheritance (Provision for Family and Dependants) Act 1975 there be paid to the Claimant out of the net estate of the deceased [a lump sum of £ ][a lump sum equal to one half of the net estate of the deceased]

() under section 7(1) of the Inheritance (Provision for Family and Dependants) Act 1975 the lump sum specified be paid by six instalments of £ each on the first day of each of the six months beginning with January

() under section 2(1)(c) of the Inheritance (Provision for Family and Dependants) Act 1975 that the property described in the attached Schedule being part of the net estate of the deceased be transferred to the Claimant

() under section 2(4) of the Inheritance (Provision for Family and Dependants) Act 1975 the Defendant who holds the above property shall execute a Conveyance / Transfer of the property in favour of the Claimant such Conveyance /Transfer to be settled by the Court in case the parties differ

() under section 2(1)(d) of the Inheritance (Provision for Family and Dependants) Act 1975 the property described in the attached Schedule being part of the net estate of the deceased be settled for the benefit of the Claimant

() under the section 2(4)(c) of the Act the Trustees of the Settlement do have the powers set out in the attached Schedule

() the (party) shall on or before (date) make an application to proceed under this Order and file a witness statement or an Affidavit exhibiting a draft settled by Counsel of a Settlement to be executed under this Order

() under section 2(1)(d) of the Inheritance (Provision for Family and Dependants) Act 1975 there be acquired out of the net estate of the above deceased at a price not exceeding £ a freehold property approved by the Claimant

() that the above property be transferred to the Claimant

() under section 2(1)(f) of the Inheritance (Provision for Family and Dependants) Act 1975 that the Settlement dated and made between (parties) be varied in the manner shown in the attached Schedule

() a memorandum of this Order be endorsed on the above Settlement

() under section 2(4) of the Inheritance (Provision for Family and Dependants) Act 1975 the (party) who holds the property described in the attached Schedule by virtue of a donatio mortis causa made by the deceased shall transfer that property to the Claimant upon receiving from the Claimant a sum equal to the capital transfer tax paid by the (party) in respect of the property

() under section 9 of the Inheritance (Provision for Family and Dependants) Act 1975 one half of the value of the severable share of the deceased in the property described in the attached Schedule at its value immediately before death be treated for the purpose of the Act as part of the net estate of the deceased
