- I hereby request that service of the claim form (or describe the other document) on (name the State) be arranged by the Foreign and Commonwealth Office under rule 6.44 of Civil Procedure Rules.
Note the applicant must file also a copy of the claim form or other document and any translation required under rule 6.45.
2. I hereby undertake personally
(a) to be responsible for all expenses incurred by the Foreign and Commonwealth Office; and
(b) to pay those expenses to the Foreign and Commonwealth Office on being informed of the amount and to produce a receipt for payment to the court office in the Central Office of the Royal Courts of Justice.
