by trustees
Please read rst
• If your application relates to the appointment
of a trustee then you need to complete this
undertaking and le it with the court together
with a COP1 application form and any other
required forms and documents. All new and
continuing trustees must sign the undertaking
(not just the person making the application).
• For further information on applications relating
to trustees please see Practice Direction F that
supplements Part 9 of the Court of Protection
Rules 2017.
• Please continue on a separate sheet of paper
if you need more space to answer a question.
Write your name, the name and date of birth of
the person to whom the application relates, and
the number of the question you are answering on
each separate sheet.
