1. (Reasons why the appointment of a new litigation friend is sought)

2. AB (Name, address and description of proposed new litigation friend) is a (state relationship if any of AB to CD) and has no interest in the matter in question in these proceedings adverse to that of CD. AB’s consent to be appointed as litigation friend is annexed.

3. AB can fairly and competently conduct proceedings on behalf of CD.

4. AB undertakes to pay any costs which CD may be ordered to pay in relation to the proceedings, subject to any right he may have to be repaid from the assets of CD.
