The forms prescribed by the Mental Health (Hospital, Guardianship and Treatment) (England) Regulations 2008 (SI 2008/1184) and the Mental Health (Hospital, Guardianship and Treatment) (England) Regulations 2012 (SI 2012/1118) for use in England by mental health professionals when performing functions under the MHA 1983 are as follows.

Form Purpose

A1 Section 2 – application by nearest relative for admission for assessment
A2 Section 2 – application by an approved mental health professional for admission for assessment
A3 Section 2 – joint medical recommendation for admission for assessment
A4 Section 2 – medical recommendation for admission for assessment
A5 Section 3 –application by nearest relative for admission for treatment
